Meet your patients’ whole-health needs with evolvedMD.
Our model significantly improves mental health outcomes, achieving remission of depression over 10x faster than traditional care.
Our model empowers PCPs and their teams to treat the whole patient, improving communication, staff well-being, and lightening the load so your providers can focus on what they’re trained to do.
Our model not only improves patient outcomes but also reduces overall total healthcare costs, driving long-term clinical and financial viability.
Limited time, resources, and mental health expertise lead 79% of PCPs to burn out, resulting in poorer quality patient care. With evolvedMD, PCPs can focus on what they do best: physical health.
How We Serve
Medical assistants, practice managers, and others play a crucial role in maintaining a high-performing practice. With evolvedMD, our model improves collaboration and curbs administrative burden.
DownloadA: Patients are best served when primary care and behavioral health work together. evolvedMD’s collaborative care model helps primary care providers treat the whole patient — addressing physical and mental health needs — right onsite at your practice.
A: evolvedMD places Behavioral Health Managers (licensed therapists and social workers) on site and in person or virtually at each of the practices we serve. With our upfront, ongoing, and in-person approach, your practice becomes a comprehensive solution for treating your patients’ physical and mental health in one place: your practice.
A: It’s as easy as Screen, Schedule, and See:
A: You will closely collaborate with the Behavioral Health Manager to deliver whole-patient care based on qualifying disorders. You will continue providing physical health services to your patients while evolvedMD’s Behavioral Health Manager, Psychiatric Consultant, and Care Coordinator will provide mental health services as necessary. You are responsible for signing off on the Behavioral Health Manager’s notes and submitting for billing as all encounters are billed under the provider.
A: The Behavioral Health Manager will chart directly into your clinic’s electronic health record. Within this system, you’ll have direct access to all documentation to assess improvement, current needs, next steps, and more. Your team can also view our clinical dashboard so you can directly see the impact with your patients. The Behavioral Health Manager will be located on site at your practice, and will be available for consultation and evaluation on care and progress if requested.
A: With insurance, the cost of a patient’s visit with our Behavioral Health Manager is similar to a visit with you, their PCP. For example, if your patient normally pays a $35 copay each time they see you, they will most likely pay the same copay each time they see the onsite Behavioral Health Manager.
