What is the ACCESS Model?
The ACCESS Model (Advancing Chronic Care with Effective, Scalable Solutions), is a 10-year program introduced by Centers for Medicare & Medicaid Services (CMS) to expand access to modern, technology-supported care for people with chronic conditions under Original Medicare.
ACCESS is designed to move beyond fee-for-service care, instead, rewarding participating organizations with “outcome-aligned payments” based on measurable health improvements.
The model will initially include four clinical tracks, targeting conditions that impact many Medicare beneficiaries, including behavioral health.
